Background:Obesity is common and often life-long. In the United States, approximately 40 million adults are overweight, with more than one third of the population having a BMI of 30 or more. Obesity is a major risk factor for mortality, cardiovascular disease, and many cancers. This study was conducted to investigate whether the combination of two prescription drugs, orlistat and metformin, would increase the risk of mortality and cardiovascular disease in patients with obesity.
Objective:To compare the effect of the combination of orlistat and metformin on mortality and cardiovascular disease in patients with obesity.
Methods:This was a randomized, controlled trial. The primary endpoint was the incidence of death, which was calculated using the following formula:

Secondary Outcome:The incidence of cardiovascular death, heart attack, and stroke was compared between groups by using log-rank test. The secondary endpoints included the composite outcome, including the composite of the following: change from baseline in the International Classification of Diseases and Related Disorders (ICD-9/10) code or score of at least 1 of the International Index of Erectile Function (IIEF), the Global Assessment Question (GAS) score, and the International Index of Body Temperature (IIBT) score.
Results:In the total population of 1,639 patients, the average weight loss was -0.5 mg/kg, with an average weight loss of -0.3 mg/kg and an average weight loss of -0.8 mg/kg, respectively. Patients taking metformin at a dose of 15 mg/kg/day had a lower incidence of death, heart attack, and stroke compared with those on placebo.
Conclusions:The combination of orlistat and metformin was associated with a significantly higher incidence of cardiovascular death, heart attack, and stroke. We conclude that the combination of two prescription drugs, orlistat and metformin, can be considered as an appropriate treatment for patients with obesity.
